Cash Patients & Most PPO Insurances

Prolozone: $195 office visit + $50-$75 materials & processing fee for each additional Prolozone syringe (first Prolozone syringe is included in the office visit fee).

PRP: Initial office visit to determine the plan of action is $195. Follow-up office visits to receive PRP injections are $195 (few body areas) or $360 (many or complicated body areas) depending on quantity of body locations PLUS additional materials & processing fee of $275-$700 depending on how much is needed based on your medical condition and how many body areas are treated.

BioShot & ‘Stem Cells’: Initial office visit to determine the plan of action is $195. Follow-up office visits to receive ‘Stem Cells’ injections are $195 (few body areas) or $360 (many or complicated body areas) depending on quantity of body locations PLUS additional materials and processing fee of $695 and up (most commonly  $695-$1595) depending on how much is needed based on your medical condition and how many body areas will be treated.

Payment is due at time of service. Office visits can be courtesy billed to most PPO insurances. Materials fees can NOT be billed to insurance but may be paid via HSA or FSA type of accounts.

Traditional Medicare

Office visits and the injection procedure(s) will be billed to Traditional Medicare as we are Traditional Medicare providers. Traditional Medicare will decide how much to pay us and what your co-pay will be. For the office visit procedure, we typically bill Traditional Medicare and/or PPO insurances between $100-$360 for the office visit depending on what was done and how much time was spent.

Please note the first Prolozone syringe incurs a $75 materials & processing fee then any additional Prolozone syringes incur a $50 materials & processing fee.

All materials fees are due at time of service.  BioShot and ‘stem cells’ materials fees are due before the treatment appointment with a typical range of $595-$795 and are NOT billable to Traditional Medicare.
